چکیده
Abstract Background Renal dysfunction is a common complication after heart transplantation (HT). replacement therapy (RRT) HT has been associated with increased risk of death. Long-term renal mainly to immunosuppressive but also strongly related post-transplant failure. Predictors early have not clearly identified. Purpose We aimed define predictors HT. Methods Our consortium includes 10 centers in the US, Canada and Europe. collected data on all consecutive single-organ recipients from 2010 2020. The primary outcome was (ERD), defined as composite need for RRT or creatinine ≥2.5 mg/dL 24 hours assessed incidence performed univariate multivariate analyses identify recipient transplant characteristics its development. Results included 2,764 recipients: 282 (10.2%) presented 2482 (89.8%) did not. Recipients who postoperative were more frequently male, Caucasian, previous sternotomy, higher baseline creatinine, longer ischemic time worse donor LVEF. They likely be under RRT, intravenous inotropes ECMO support there severe graft (PGD) (Table 1). Multi-variable logistic regression demonstrated that strongest development PGD (OR 5.26, 2.88–9.62, p<0,001) prior 5.80, 2.93–11.5, p<0.001). Other male sex, long Conclusions Early an around 10% large contemporary cohort. presence pre-transplant Interestingly, emergent MCS independently ERD. Primary Graft Dysfunction After Lung Transplantation.
Primary graft dysfunction is a form of acute injury after lung transplantation that is associated with significant short- and long-term morbidity and mortality. Multiple mechanisms contribute to the pathogenesis of primary graft dysfunction, including ischemia reperfusion injury, epithelial cell death, endothelial cell dysfunction, innate immune activation, oxidative stress, and release of infl...
متن کاملLong Term Impact of Primary Graft Dysfunction after Lung Transplantation
According to the International Society for Heart and Lung Transplantation (ISHLT) Registry, graft failure accounted for 24.7% of deaths within 30 days of transplantation among adult lung recipients between 1992 and 2012 [1]. Although important details distinguishing different causes of early graft failure may be missing from the Registry data, it is likely that primary graft dysfunction (PGD) i...
متن کاملPercutaneous Extracorporeal Membrane Oxygenation for Graft Dysfunction after Heart Transplantation
BACKGROUND We evaluated the safety and efficacy of percutaneous extracorporeal membrane oxygenation (ECMO) in patients with primary graft dysfunction after heart transplantation. METHODS Of 65 patients (44 males and 21 females) who underwent heart transplantation from January 2006 to December 2012, 13 patients (group I) needed peripheral ECMO support due to difficulty in weaning from cardiopu...

We read with interest the report by Aptecar et al. (1) that early after heart transplantation, coronary vasodilator effects of bradykinin are preserved despite abnormal response to intracoronary injection of acetylcholine. This absence of alterations to the vasodilator action of bradykinin is neither a new nor an unexpected finding. In cardiac transplant recipients with normal angiography, this...
متن کاملExperience of ECMO in Primary Graft Dysfunction after Orthotopic Heart Transplantation
BACKGROUND Primary graft dysfunction is the main cause of early mortality after heart transplantation. Mechanical circulatory support has been used to treat this syndrome. OBJECTIVE Describe the experience with extracorporeal membrane oxygenation to treat post-transplant primary cardiac graft dysfunction. METHODS Between January 2007 and December 2013, a total of 71 orthotopic heart transpl...
